Any recommendations on facilities you have used in the past with success, for plant tissue testing?
A&L Western doesn't do them anymore. They e-mailed me today, to let me know.

Are you looking for nutrients or disease? (Your title suggest disease and your details suggest nutrients.)
If you are looking for disease I would ask the follow-up question of what type of disease? (bacteria, fungi, and/or virus)

Matt, I suspect fusarium but it could be Mn deficiency. What would be the best way of testing?

I contacted A&L via e-mail. They e-mailed me back and said they were no longer testing plant samples. Maybe they have too much of a back log or something.
