When pruning on the secondary vines, do I cut the tendril and tertiary off and leave the main leaf and the flower?

Yes, unless you have some space left that the tertiary vine could grow in.

Cut the flowers as well unless you need them for future pollinations...

tendrils can help hold your plant in place. if you have a solid fruit set cut all other flowers off.
